I hereby certify that the facts contained in this submission are true and complete to the best of my knowledge. I understand that any misrepresentations, omissions, or falsifications of this information may prevent me from performing contracted labor for the company and can be grounds for immediate denial of entry my or removal from consideration or, if I have entered into a contract with The Moving Firm LLC, for immediate termination of that contract.
I authorize any necessary inquiry and investigation of all statements contained herein regarding the references, employers, and/or contacts listed above to give The Moving Firm LLC any and all pertinent information (personal or otherwise) concerning my previous employment, and release them from all liability for any damage that may result from utilization of such information.
I also hereby release from liability The Moving Firm LLC and its representatives for seeking, gathering, and using such information to make decisions concerning my status as an Independent Contractor in their contractor network, and all other persons or organizations for providing such information.
I also understand and agree that no representative of the Company has any authority to enter into any contracting agreement for any specified period of time, or to make any agreement contrary to the foregoing, unless it is in writing and signed by an authorized representative.
THIS IS NOT AN APPLICATION FOR EMPLOYMENT. I understand and agree that if this application is accepted, my status will be that of an Independent Contractor and as such, I will be solely responsible for all tax liabilities pertaining to monies received in the course of services I perform.
If I am accepted to join the The Moving Firm LLC Independent Contractor Network I will:
~ Not be entitled to workers compensation benefits.
~ Not be entitled to unemployment insurance benefits unless unemployment coverage is provided by me or some
other entity.
~ Be obligated to pay federal and state income tax on any moneys paid pursuant to the contract.
~ Be required to provide professional and liability insurance.
I represent and warrant that I have read and fully understand the foregoing, and that I seek to join The Moving Firm LLC's Independent Contractor Network under these conditions.
